This is a repair of a complex medial meniscal tear in a young patient. There are horizontal cleavage and flap components. Repair was performed using all inside and circumferential suturing, maintaining maximal meniscus volume
This is a root adjacent tear. The root remainder is too fragile and small for a full hashtag repair. It is repaired with the transitibial tunnel technique augmented with two side to side sutures.
Centralisation of medial meniscal root tear repair is an developing (and somewhat controvertial) field with indications still not fully evolved. In addition, current techniques vary considerably . Where the surgeon feels centralisation is needed, this is a simplified transtibial approach with the use of two all inside anchors to repair the lax meniscotibial ligament.
Young (20's), very active patient -8 months of conservative management -widely displaced radial LM tear with cartilage change in the lateral compartment -slight mild varus alignment. Very challenging and on the limit of what can be done to repair.
This is an unusual tear pattern. Sort of a LMORT of the anterior horn. Repair using a circumferential suture technique, shuttling the inferior sutures via bird-beak . Spinal needle shuttling technique would work well here. Final construct is solid. Restrict full extension for 4 weeks post- op
